The following forms and documents apply to camps, clinics, training sessions, programs, and events organized or operated by CURRANOLOGY, LLC.

Parents and legal guardians are encouraged to review all applicable documents before registering a participant.

When registering online, parents/legal guardians will be asked to review and electronically acknowledge the applicable documents.

If you complete registration online, you do not need to print, sign, or return these documents separately.

This agreement outlines important information regarding participation, assumption of risk, release of liability, participant health and safety, emergency medical authorization, concussion and head-injury procedures, and other terms applicable to CURRANOLOGY, LLC activities.
Please note: Master Participation Agreement is required for participation.
